The World Health Organization has published an updated framework for AI governance in health systems, providing guidance for governments, ministries of health and health authorities on how to manage the adoption of AI technologies in clinical and public health settings.
The framework covers the full lifecycle of AI in health: from procurement and pre-deployment evaluation through to post-market surveillance, incident reporting and decommissioning. It emphasises that governance structures must be established before AI tools are deployed, not retrofitted after problems emerge.
A significant portion of the guidance addresses equity. The WHO notes that AI systems trained predominantly on data from high-income countries may perform less well in lower-income settings, and that health systems with less digital infrastructure face particular challenges in evaluating and monitoring AI tools.
The framework also addresses the role of patients and communities in AI governance, arguing that affected populations should have meaningful input into decisions about which AI tools are used in their care and how those tools are evaluated.
The WHO guidance is non-binding. Its practical influence depends on whether national health authorities incorporate its recommendations into their own regulatory and procurement frameworks.
